What you will do:

• Conduct Care Act 2014 assessments and reviews for individuals with complex needs.

• Arrange the provision of necessary services to meet eligible needs.

• Work in a person-centred and strengths-based manner with clients and their families.

• Undertake safeguarding responsibilities and ensure protective measures.

• Conduct capacity assessments in line with the Mental Capacity Act 2005.

• Participate as a duty worker on a rotating basis, providing support as needed.

• Utilize your experience to contribute to team goals and improve service delivery.

Our office is located conveniently in Ilkeston, with an expectation that new workers attend the office for the first two weeks for a comprehensive induction. After this period, you will enjoy a hybrid working model that allows you to work both from home and at the office, tailored to service needs. On duty days, you'll be based at the office to ensure seamless duty operations.
