What you will do:
- Manage your own caseload effectively, working with individuals living in the community across Hertfordshire.
- Undertake holistic assessments to identify person-centred outcomes and promote meaningful occupation.
- Collaborate closely with families and carers to encourage independence and support through a strengths-based approach.
- Align with the Adult Care Services Connected Lives practice principles to connect people to the life they aspire to lead.
- Work alongside social care colleagues, health partners, housing and care providers, and community support services.
- Assess needs for aide adaptations and ensure safe transfers and repositioning.
- Engage in hybrid working, combining in-person and remote tasks.
To excel in this role, it is essential to be registered with the HCPC and be a car driver, as navigating Hertfordshire will be a key part of your responsibilities.
