What you will do:

- Provide high-quality nursing care and support to patients in the community setting.
- Conduct comprehensive assessments and develop personalised care plans.
- Deliver efficient and compassionate healthcare to elderly patients with complex needs.
- Collaborate with multi-disciplinary teams (MDT) to ensure holistic patient care.
- Utilize System 1 for patient documentation and communication.
- Perform occasional home visits to deliver care in a familiar environment for patients.
- Educate patients and families on health management and disease prevention.
- Participate in team meetings and contribute to the community health strategy.

Requirements:

- Previous experience working as a Band 6 Nurse within the NHS is essential.
- Broad expertise in community nursing, particularly with elderly patients with complex health needs.
- Experience collaborating in an MDT environment.
- Proficiency in System 1 is advantageous.
- A valid driver's license and access to a car are preferred, although not essential.
