What you will do:

  • Conduct complex person-centred assessments, including those related to mental capacity, to support individuals in achieving the highest level of independence possible.
  • Provide expert advice and support to carers in managing challenging situations.
  • Accurately record and manage social care data in compliance with organizational policies.
  • Engage with multi-disciplinary teams, offering expert opinions and acting as a liaison between professionals.
  • Guide and mentor junior staff, providing supervision and support to newly qualified social workers.
  • Organize and chair meetings to fulfil legal requirements under the Mental Capacity Act 2005.
  • Undertake safeguarding responsibilities and act as an Investigating Officer in accordance with local policies.
  • Develop and coordinate support plans in collaboration with individuals, families, and external partners.
