What You Will Do:
Conduct in-depth care management assessments and reviews to determine the outcomes important to the service users and their carers.
Encourage service user independence, recovery, and enablement by connecting them with community-based services.
Craft personalised, creative support plans that promote choice and control, focusing on the use of personal budgets and Direct Payments.
Manage risk diligently, following safeguarding procedures for adults and coordinating with Children’s Social Care when necessary.
Oversee case management for adults needing ongoing specialist intervention.
Work closely with multi-disciplinary teams, including health professionals and community development staff, to provide integrated support.
Ensure thorough and accurate record-keeping in accordance with organisational policies and legal standards.
Person Specification:
Qualified Social Worker registered with Social Work England.
Completion of the Assessed and Supported Year in Employment (ASYE) with at least two years of post-qualification experience.
Demonstrated experience working with adults with learning disabilities, ADHD, and autism.
Strong understanding of relevant legislation such as the Care Act 2014, Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS), Safeguarding Adults, and the Equality Act.
Proficient in strengths-based assessment and review techniques.
Exceptional communication and interpersonal skills to engage effectively with service users, carers, and professionals.
A commitment to enhancing service users' independence, choice, and control.
Capability to manage complex cases under pressure.
Training and Development:
Engage in in-house training and team learning events aimed at Continuing Professional Development (CPD).
