What you will do:
- Conduct holistic assessments of social care needs and plan support across a dedicated caseload.
- Work closely with families, carers, and care workers to support individuals from diverse backgrounds.
- Assess for major adaptations and specialised equipment, and complete moving and handling risk assessments.
- Utilise your professional expertise in diverse cases, applying knowledge of current legislation, such as the Care Act and Mental Capacity Act.
- Provide professional supervision and guidance to other team members to maintain high-quality service delivery.
