In this role, specifically, you'll collaborate with Occupational Therapists and Social Care Co-ordinators, giving you the unique chance to engage in interdisciplinary learning and improve holistic care delivery.

What you will do:

• Conduct Care Act assessments and develop tailored support plans for adults aged 18+ with physical disabilities and those over 65.
• Collaborate closely with partner agencies to ensure coordinated and comprehensive care.
• Handle adult safeguarding processes with confidence and diligence.
• Perform Mental Capacity Assessments and engage in relevant court work, including Court of Protection applications.
• Apply your solid understanding of relevant legislation to guide your practice and decision-making.
• Communicate effectively with clients, families, and team members while contributing positively to our team dynamic.
