What you will do:

• Actively contribute as a member of the clinical team, providing specialised falls services to residents in Westminster, Kensington, Chelsea, and Hammersmith.
• Conduct comprehensive multi-factorial falls assessments and deliver evidence-based rehabilitation programmes for clients who have experienced falls.
• Manage a clinical caseload of patients at risk of falls and those needing community-based rehabilitation, implementing client-centred, evidence-based physiotherapy interventions.
• Adhere to professional standards, work autonomously, assess and treat your caseload, and maintain professional documentation.
• Build and maintain key relationships with referral services, enhancing collaboration with other healthcare providers and service teams.
• Supervise junior staff, Rehabilitation Assistants, and Physiotherapy students whenever possible.
• Provide services in diverse settings, including clinics, clients' homes, community centres, sheltered housing, and care homes.
• Support personal and service development through Continuing Professional Development and participation in audits or research.

Requirements:

• Current registration with the Health and Care Professions Council (HCPC) is essential.
• Minimum of two years' experience in physiotherapy, preferably with expertise in falls prevention.
• Being a car driver would be advantageous.
