• Deliver recovery-focused interventions to service users within the community, ensuring personalised and effective care.
• Coordinate CPA care plans and assessments, including Care Act Assessments, to support the social work needs of service users.
• Conduct specialist assessments and interventions, providing expert guidance to team members on social difficulties.
