What you will do:

  • Conduct comprehensive assessments and develop detailed support plans aimed at discharging care orders via SGO.

  • Provide regular support, advice, and mentorship to less experienced team members, fostering a collaborative atmosphere within the team.
  • Manage a select caseload of five families, planning and executing tasks in an organized and effective manner.
