What you will do:

• Carry out person-centred occupational therapy assessments within the community or residential settings.
• Recommend and arrange the provision of appropriate equipment and minor adaptations to support independent living.
• Contribute to complex moving and handling plans and provide training to carers or support workers where needed.
• Work collaboratively with service users, carers, families, and other professionals, including social workers and healthcare providers.
• Provide timely and clear documentation, including reports and support plans, in line with council procedures and legal frameworks.
• Support discharge planning from hospital to community settings, ensuring safe transitions and minimal delays.
• Assist in safeguarding investigations and contribute to multi-disciplinary meetings where appropriate.
• Promote preventative strategies and strengths-based approaches to care.

Requirements:

• Degree/Diploma in Occupational Therapy
• Current registration with the Health and Care Professions Council HCPC
• Previous experience in adult social care or community occupational therapy
• Good understanding of the Care Act 2014, Mental Capacity Act, and related legislation
• Strong communication, assessment, and problem-solving skills
• Enhanced DBS check on the update service or willing to obtain
• Ability to travel independently across the borough
