What you will do:
- Carry out person-centred occupational therapy assessments within the community or residential settings.
- Recommend and arrange the provision of appropriate equipment and minor adaptations to support independent living.
- Contribute to complex moving and handling plans and provide training to carers or support workers where needed.
- Work collaboratively with service users, carers, families, and other professionals, including social workers and healthcare providers.
- Provide timely and clear documentation, including reports and support plans, in line with council procedures and legal frameworks.
- Support discharge planning from hospital to community settings, ensuring safe transitions and minimal delays.
- Assist in safeguarding investigations and contribute to multi-disciplinary meetings where appropriate.
- Promote preventative strategies and strengths-based approaches to care.
Requirements:
- Degree/Diploma in Occupational Therapy
- Current registration with the Health and Care Professions Council HCPC
- Previous experience in adult social care or community occupational therapy
- Good understanding of the Care Act 2014, Mental Capacity Act, and related legislation
- Strong communication, assessment, and problem-solving skills
- Enhanced DBS check on the update service or willing to obtain
- Ability to travel independently across the borough
