What you will do:
- Undertake statutory social work responsibilities under the Care Act 2014, including Care Act Assessments, Best Interest Decisions (BID), and Mental Capacity Assessments (MCA).
- Manage a caseload of young adults with Learning Disabilities (LD), Autism, ADHD, Global Developmental Delay (GDD), and other physical/sensory impairments.
- Facilitate seamless transitions for young adults leaving Children's Services, offering continuity of care and personalized support.
- Collaborate with partner agencies such as SALT, OTs, LD Nurses, Psychologists, Behaviour Therapists, and Psychiatrists.
- Conduct holistic needs assessments, risk assessments, safeguarding enquiries, care planning, and reviews within statutory frameworks.
- Advocate for service users, promoting independence, social inclusion, and access to appropriate services.
- Attend multidisciplinary team meetings and contribute to case discussions and planning.
- Maintain clear, concise, and up-to-date records in accordance with professional and legal requirements.
- Provide support, advice, and guidance to families and carers throughout the transition process.
Essential Requirements:
- Qualified Social Worker with current Social Work England registration.
- Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and related legislation.
- Experience working with young adults with LD, Autism, ADHD, and associated conditions.
- Excellent communication, assessment, and organisational skills.
- Proven ability to work both independently and as part of a multi-agency team.
- Commitment to promoting equality, diversity, and anti-discriminatory practice.
Desirable:
- Experience within a transitions or adult learning disability team.
- Familiarity with Hackney’s social care systems, resources, and pathways.
- Knowledge of local authority procedures and safeguarding frameworks.
