What you will do: Manage a defined caseload of adults with complex and enduring mental health needs, acting as Care Coordinator where appropriate.Undertake comprehensive mental health, risk and needs assessments, formulating and reviewing person-centred care plans.Deliver evidence-based therapeutic interventions and recovery-focused treatments within community settings and patients' homes.Monitor, assess and manage clinical risk, including safeguarding concerns, ensuring timely escalation where necessary.Work collaboratively with psychiatrists, psychologists, occupational therapists, social workers, primary care services, and voluntary sector partners. Person Specification: Hold qualifications as a Registered Mental Health Nurse (RMN) with either a degree or diploma in Mental Health Nursing.Demonstrate evidence of continuing professional development relevant to community mental health practice.Possess mentorship, supervision, or leadership training, which will be considered desirable.Maintain current registration with the Nursing and Midwifery Council (NMC).Have significant post-registration experience within adult mental health services and experience working within a Community Mental Health Team, Crisis Service, Early Intervention Service, or similar secondary mental health setting.
