It covers the type of care needed, how the care will be given, and how often the care will be carried out. The care plan will need to contain a lot of person-specific information. This not only includes their medical needs, multi-agency contacts, allergies and any risk assessments, but the extra details that make sure we can provide the best service possible. This can include our client’s likes and dislikes, and how they prefer to be addressed. What You Will Do Support Intermediate Care Physiotherapists in delivering high-quality therapy and rehabilitation services. Assist patients with therapeutic exercises and mobility programmes tailored to their individual needs. Maintain accurate, up-to-date patient records and document treatment progress. Work collaboratively with a multidisciplinary healthcare team to deliver holistic patient care. Contribute to planning and reviewing therapeutic activities during team meetings. Provide patient feedback and observations to help inform treatment planning. Adhere to all relevant health and safety policies and procedures. Requirements Preferably HCPC registered, with at least 2 years’ experience in a relevant therapy or physiotherapy support role. A full UK driving licence is desirable for travelling between sites.
