Conduct medicines reconciliation and assess patients' own medicines for newly admitted patients, resolving any issues and documenting them accurately on electronic patient records. Manage the ordering of medicines for both patients and ward stock. Attend weekly team meetings to support patient care during their hospital stay and facilitate discharge preparation. Assist patients and nursing staff with self-administration of medicines to promote rehabilitation and independence. Collaborate with community pharmacies as needed on patient admission and discharge. Requirements: Registration: Registration with the Health and Care Professions Council (HCPC) is essential. Experience: At least 2 years of experience in a similar role is required.
