What you will do

Case Management: Manage a varied caseload of young people transitioning from Children's Services to Adult Social Care.
Assessment: Undertake strengths-based assessments in accordance with the Care Act 2014.
Care Planning: Develop, implement, and review personalised care and support plans that reflect individual needs, aspirations, and outcomes.
Transition Planning: Work collaboratively with Children's Services, Adult Social Care teams, and partner agencies to ensure smooth and effective transition arrangements.
Family Support: Provide guidance and support to young people and their families regarding available services, resources, and funding options.
Mental Capacity Assessments: Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.
Safeguarding: Identify, manage, and respond appropriately to safeguarding concerns.
Advocacy: Promote independence, inclusion, self-determination, and person-centred practice.
Multi-Agency Working: Participate in multidisciplinary meetings, transition reviews, and safeguarding processes where required.
Record Keeping: Maintain accurate, confidential, and timely records using electronic case management systems.
Compliance: Ensure all work is completed in accordance with statutory duties, council policies, and professional standards.

Qualifications / Requirements

Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
Registration: Current Social Work England registration.
Experience: Previous experience within Adult Social Care, Children's Services, or Transitions Services.
Legislative Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, safeguarding frameworks, and relevant legislation.
Assessment Skills: Experience undertaking strengths-based assessments and person-centred care planning.
Communication Skills: Excellent written, verbal, and interpersonal communication abilities.
Partnership Working: Ability to work effectively with families, carers, and multidisciplinary professionals.
Organisational Skills: Strong case management and prioritisation skills within a busy service environment.
