What you will do:

• Conduct brief interventions and assessments to facilitate effective support plans.
• Undertake core assessments to identify clients' needs and ensure their well-being.
• Provide short-term keyworking, offering clients the immediate support they require.
• Deliver long-term keyworking under the Care Programme Approach (CPA) for clients requiring sustained assistance.
• Support the team by covering the Duty team, ensuring no client goes without attention.
• Collaborate within an integrated health and social community mental health team to provide seamless, holistic care.
