What you will do:

• Undertake strengths-based assessments of adults' needs and risks, ensuring a thorough and empathetic approach.

• Provide early intervention and preventative support to reduce escalation and foster autonomy.

• Develop and review care and support plans that promote independence, while aligning with the Care Act 2014 and relevant legislation.

• Support individuals in accessing essential community resources and services, ensuring they receive comprehensive assistance.

Person specification:

Qualifications:

• Degree or equivalent qualification in Social Work

Registration & Compliance:

• Registered with Social Work England

• Enhanced DBS

• Right to Work in the UK

Key Skills:

• Strong assessment and early intervention skills with the ability to implement preventative measures effectively

• Excellent understanding of Adult Social Care legislation, including the Care Act

• Proven ability to promote independence through strengths-based practice

• Exceptional communication and partnership working skills

• Solid organisational and case management capabilities

• Ability to work autonomously and collaboratively within a team setting

Experience:

• Post-qualification experience in Adult Social Care

• Experience in early help, prevention, or community-based services

• Demonstrable experience supporting adults with a variety of needs

• Experience of multi-agency working and effective collaboration
