What you will do

• Assessment and Support Planning: Undertake strengths-based and needs-led assessments, developing personalised care and support plans.
• Hospital Admission Prevention: Support adults and carers through interventions designed to reduce unnecessary hospital admissions and promote community-based care.
• Safeguarding: Conduct safeguarding enquiries in accordance with the Care Act 2014, ensuring individuals receive appropriate protection and support.
• Multi-Agency Working: Collaborate with health professionals, community teams, carers, and partner organisations to deliver coordinated services.
• Communication: Effectively communicate complex information to individuals, carers, and professionals using a range of methods and approaches.
• Meeting Coordination: Organise, chair, and contribute to multidisciplinary meetings, ensuring accurate records and action plans are maintained.
• Advocacy: Promote the rights, independence, and wellbeing of adults, challenging discrimination and barriers linked to age, disability, or mental health needs.
• Community Engagement: Participate in community initiatives and forums to improve service delivery and strengthen local support networks.
• Record Keeping: Maintain accurate, timely, and compliant case records in line with statutory and organisational requirements.

Qualifications / Requirements

• Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
• Registration: Current Social Work England registration.
• Experience: Previous experience within Adult Social Care, community services, hospital discharge, or integrated health and social care settings.
• Legislative Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, safeguarding procedures, and related legislation.
• Assessment Skills: Experience undertaking strengths-based assessments, care planning, and risk management.
• Communication Skills: Excellent written, verbal, and interpersonal communication abilities.
• Partnership Working: Ability to work effectively within multidisciplinary and multi-agency environments.
• Organisational Skills: Strong case management and prioritisation skills within a fast-paced service.
