What you will do:

- Lead the clinical management of care packages for individuals using our services.
- Coordinate and monitor the delivery of person-centred healthcare for a designated group of clients, using complex clinical decision making where appropriate.
- Deliver individual and group therapy utilising CBT skills.
- Work as a member of the multidisciplinary team in assessing and planning care, and respond to changes in the health of people using our services.
- Ensure the provision of necessary physical care, including personal hygiene, diet and fluid intake, clothing and laundry, personal environment, manual handling, elimination, maintaining rest and sleep, and activity.
- Act respectfully, with a commitment to supporting individuals' customs, values, sexuality, and spiritual beliefs.
- Provide therapeutic activities, emotional support, and stability to clients, ensuring effective engagement while maintaining professional boundaries.
- Build and maintain supportive relationships with staff, other professionals, and agencies to enhance recovery.
- Undertake prescribed physical observations, including blood pressure, temperature, pulse, respirations, blood sugars, urinalysis, weight, and height, and act appropriately on any exceptions.

Requirements:

- NMC registration is required.
- A car driver is essential to reach various community locations and ensure efficient delivery of care.
