What you will do:
- Deliver a professional social work service to adults, their families, and carers in accordance with the Care Act 2014, focusing on wellbeing and significant impact identification.
- Ensure compliance with statutory requirements in mental health legislation, adeptly managing and evaluating risk while applying the Mental Capacity Act 2005.
- Assist individuals, carers, and families by conducting strength-based assessments to identify personal, community strengths and necessary services.
- Develop support plans aimed at achieving personalised outcomes with a preventative approach, reducing and delaying needs whilst offering essential advice and informed decision-making support through advocacy arrangements.
- Undertake safeguarding enquiries aligned with legislation, ensuring the adult remains at the heart of the process, while working holistically and collaboratively with other professionals.
