This full-time role offers an hourly rate of £32 for an initial contract of 3 months. The position requires you to dive into engaging and meaningful work, specifically handling hospital discharges and managing complex adult mental health cases within the local authority's mental health team.

What you will do:

• Manage complex adult mental health cases and coordinate seamless hospital discharge processes.
• Conduct statutory Care Act tasks, including Care Act assessments, reviews, and care planning.
• Work effectively alongside multi-disciplinary professionals, attending ward rounds and collaborating with Housing and brokerage services.
• Handle Court of Protection applications and take part in Mental Health Tribunals.
• Carry out Safeguarding Enquiries and perform duty work efficiently.
• Travel around and beyond the borough as needed, requiring you to have a valid driver's license and your own transport.
• Thrive in a fast-paced environment by proactively managing and prioritising workloads, ensuring urgent cases receive the attention they require.
