This full-time role offers an hourly rate of £32 for an initial contract of 3 months. What you will do:
- Manage complex adult mental health cases and coordinate seamless hospital discharge processes.
- Conduct statutory Care Act tasks, including Care Act assessments, reviews, and care planning.
- Work effectively alongside multi-disciplinary professionals, attending ward rounds and collaborating with Housing and brokerage services.
- Handle Court of Protection applications and take part in Mental Health Tribunals.
- Carry out Safeguarding Enquiries and perform duty work efficiently.
- Travel around and beyond the borough as needed, requiring you to have a valid driver's license and your own transport.
- Thrive in a fast-paced environment by proactively managing and prioritising workloads, ensuring urgent cases receive the attention they require.
