What you will do:

- Deliver high-quality physiotherapy services within the community, focusing on rehabilitation and improving patient outcomes.
- Conduct assessments and develop individualised treatment plans for patients with varying conditions.
- Collaborate with multidisciplinary teams, patients, and families to ensure comprehensive care and optimal recovery.
- Record and report on patient progress and adapt treatment plans as necessary to meet evolving patient needs.
- Educate patients and their families on exercises and techniques to support home rehabilitation efforts.

Requirements:

- Previous experience in community rehabilitation is essential.
- Must be a registered HCPC physiotherapist with a minimum of two years of experience.
- A full UK driving licence and access to a car are required, ensuring you can effectively travel across the community to meet patients.
