What you will do:

  • Manage a caseload of care-experienced young people aged 16–25, focusing on independence preparation and pathway planning

  • Complete and regularly review Pathway Plans, ensuring alignment with statutory guidance and individual needs

  • Identify and respond to safeguarding concerns, undertaking risk assessments and contributing to robust safeguarding plans

  • Facilitate smooth transitions from Children in Care (LAC) teams to Leaving Care services, promoting stability and positive outcomes into adulthood

  • Work collaboratively with multi-agency partners, ensuring cohesive support for education, housing, wellbeing, and more

  • Advocate for young people, centring their views and ensuring engagement through trusted relationship-building

  • Maintain precise case records and participate in ongoing training and development
