What you will do:
- Conduct comprehensive Care Act assessments and develop personalised support plans for adults and their carers.
- Facilitate timely hospital discharges by coordinating multi-agency collaborations to ensure safe and effective outcomes.
- Apply your knowledge of statutory social work, the Mental Capacity Act, and Best Interest decision-making to safeguard vulnerable individuals.
- Engage with adults, carers, and other professionals to optimise care solutions within a demanding fast-paced hospital setting.
- Tackle safeguarding responsibilities under Section 42 with confidence and expertise.
