What you will do:

• Conduct Safeguarding Section 42 enquiries to ensure the safety and wellbeing of vulnerable adults.
• Join and collaborate in safeguarding huddles with the Hospital Safeguarding lead to address urgent concerns and protect at-risk individuals.
• Manage a complex caseload with diligence, which includes crafting court of protection applications and undertaking Section 21a Applications.
• Support patients through reablement, promoting quicker recovery and aiding their return to independence.
• Work closely with multidisciplinary teams in hospitals to create and maintain effective care pathways.
• Liaise with families and carers, providing guidance and support during challenging times.
