What you will do:

- Facilitate safe and timely hospital discharges by assessing patients' social care needs and devising comprehensive discharge plans.
- Collaborate with multidisciplinary teams, including medical staff, families, and external agencies, to ensure cohesive care planning.
- Conduct assessments and reviews, ensuring compliance with relevant guidelines and frameworks.
- Provide emotional and practical support to patients and their families, navigating through challenging transitions.
- Maintain accurate and detailed documentation in line with organisational policies and procedures.

This role must align with MOC guidelines including:

- Candidates must have two years of post-qualified experience.
- Applicants should have refrained from permanent social worker roles for the past 12 months.
- A one-month notice period is required for this position.
- The role offers a rate of £28.40 per hour through an umbrella company, inclusive of HPIL.
