What you will do
- Assessment & Care Management: Provide comprehensive assessments and care planning for adults within the CMHT setting.
- Multidisciplinary Working: Collaborate with health professionals, families, and community organisations to deliver holistic support.
- Care Planning: Develop and implement person-centred care plans that promote independence and wellbeing.
- Crisis Intervention: Respond effectively to crisis situations, supporting individuals to manage risks and make informed decisions.
- Case Recording: Maintain accurate, up-to-date records in line with statutory and organisational requirements.
Qualifications / Requirements
- Qualified Social Worker with current Social Work Wales registration.
- Experience working within adult social care, ideally within a mental health setting.
- Strong knowledge of relevant legislation and safeguarding frameworks.
- Ability to manage a caseload independently and work within a multidisciplinary team.
