What you will do:

Undertake strengths-based assessments of adults’ needs, risks and outcomes.
Deliver early intervention and preventative support to avoid crisis or long-term care.
Develop and review care and support plans promoting independence.
Work in accordance with the Care Act 2014 and relevant legislation.
Support individuals to access local community services and resources.

Person specification:

Qualifications:

Degree or equivalent qualification in Social Work

Registration & Compliance:

Registered with Social Work England
Enhanced DBS
Right to Work in the UK

Key Skills:

Strong assessment and early intervention skills
Good understanding of Adult Social Care legislation, including the Care Act
Ability to work in a strengths-based and person-centred way
Excellent communication and partnership working skills
Strong organisational and case management abilities
Ability to work independently and within a team

Experience:

Post-qualification experience in Adult Social Care
Experience within early intervention, prevention or locality teams
Experience supporting adults with a range of needs
Experience of multi-agency partnership working
