What you will do:

  • Undertake strengths-based assessments of adults’ needs, risks and outcomes.

  • Deliver early intervention and preventative support to avoid crisis or long-term care.

  • Develop and review care and support plans promoting independence.

  • Work in accordance with the Care Act 2014 and relevant legislation.

  • Support individuals to access local community services and resources.

Person specification:

Qualifications:

  • Degree or equivalent qualification in Social Work

Registration & Compliance:

  • Registered with Social Work England

  • Enhanced DBS

  • Right to Work in the UK

Key Skills:

  • Strong assessment and early intervention skills

  • Good understanding of Adult Social Care legislation, including the Care Act

  • Ability to work in a strengths-based and person-centred way

  • Excellent communication and partnership working skills

  • Strong organisational and case management abilities

  • Ability to work independently and within a team

Experience:

  • Post-qualification experience in Adult Social Care

  • Experience within early intervention, prevention or locality teams

  • Experience supporting adults with a range of needs

  • Experience of multi-agency partnership working
