Join our team as a Social Worker within the Community / Home First Team in the beautiful region of East Lothian. What You Will Do:

• Undertake SDS (Self-Directed Support) assessments for community-based cases involving adults aged 16+, including those experiencing frailty and older adults.

• Carry out community interventions, assessments, and long-term care planning, including care home placements where appropriate.

• Work collaboratively within a multidisciplinary team to prevent unnecessary hospital admissions and support effective discharge planning.

• Travel across East Lothian to complete community visits, using your own vehicle due to the rural nature of the area.

• Contribute to short-term intervention planning and Home First initiatives to promote independence and wellbeing.

This role is primarily based at John Muir House, Haddington, with some work across East Lothian Community Hospital and occasional visits to the Royal Infirmary of Edinburgh.
