Purpose of the Job:
Work with adults with complex needs to assess their circumstances in accordance with The Care Act using a Strength Based and Enabling approach. Formulate, co-ordinate and monitor support plans where necessary to promote independence. Collaborate with adults, carers, family members, professionals, and others ensuring a person-centred approach. Provide social work interventions in response to safeguarding concerns, including safeguarding enquiries under the guidance of a line manager.

What you will do:
- Assess the needs of adults with complex needs using a Strength Based approach.
- Develop and coordinate support plans to encourage independence.
- Collaborate effectively with carers, professionals, and family members.
- Conduct safeguarding enquiries and address concerns under guidance.
- Maintain professional and ethical boundaries in social work practice.

Requirements of the Social Worker:
- Degree or equivalent in social work.
- Current Social Work England registration.
- Significant frontline experience in UK Social Services as a qualified Social Worker.
