What you will do:

• Carry out assessments of adults’ social care needs, identifying risks, strengths, and appropriate interventions

• Develop, implement, and review care and support plans that promote independence and wellbeing

• Work collaboratively with internal teams and external partner agencies to coordinate effective care packages

• Provide advice, guidance, and support to individuals and their families navigating complex situations

• Maintain accurate and up-to-date case records, ensuring confidentiality and data protection compliance

About you / requirements:

• Qualified Level 3 Social Worker with relevant post-qualifying experience

• Proven ability to manage a highly complex adult social care workload

• Strong assessment, communication, and decision-making skills

• Resilient, organised, and confident working within a fast-paced locality team
