What you will do:
- Conduct comprehensive assessments in line with the Care Act to ensure individuals receive appropriate support and care.
- Work within the community setting, focusing on safeguarding adults, and undertaking care management functions.
- Engage in the NHS Continuing Healthcare assessments and collaborate with multidisciplinary teams to ensure best outcomes for service users.
- Present cases to the Outcomes Forum for funding decisions, showcasing your analytical and persuasive skills.
- Participate in strategic planning and development of personalised care plans.
- Attend the office three times a week to collaborate with a passionate team, enabling shared learning and experience exchange.
