What you will do:
- Deliver specialized occupational therapy interventions to stroke patients within the community.
- Conduct comprehensive assessments of patients' needs and develop tailored treatment plans.
- Collaborate with multidisciplinary teams to deliver outstanding patient-centred care.
- Monitor and evaluate patient progress, adjusting therapies and interventions as required.
- Provide education and support to patients and their families about stroke recovery and rehabilitation processes.
Requirements:
- Previous experience within a community-based stroke service is advantageous.
- Proficiency with SystmOne is desirable but not essential, offering you a chance to learn on the go.
- You must be a registered member of the HCPC, ensuring high standards of practice.
- A valid driver's license and access to your own car are required as you will be covering the Hull and East Riding areas.
