What You Will Do:
- Provide a person-centred, statutory social work service for vulnerable adults and their carers, aligned with local and national policies.
- Actively support and participate in integrated, multi-disciplinary working to benefit clients.
- Take a key role in safeguarding, managing risk, and sharing information with partners.
- Use a strengths-based approach to develop effective support plans for people with lived experience.
- Manage your caseload efficiently, carrying out social care assessments and reviews to promote independence and well-being.
- Develop and maintain relationships with service providers and voluntary groups to support community-led social care.
- Produce and agree support plans with individuals and their families, ensuring choice and participation in service planning.
Organisational and Professional Development:
- Contribute to the advancement of team services through specialised roles and project work.
- Undertake training to enhance professional skills, such as Best Interest Assessor training.
- Mentor and support other team members and students in their professional development.
- Stay updated with legal, policy, and procedural frameworks to contribute to team improvement.
