What you will do:

• Conduct comprehensive adult needs assessments, ensuring that care plans are tailored to individual requirements.
• Utilise your extensive knowledge of Mosaic to manage cases effectively across the South area of Lincolnshire, with office bases flexible in locations such as Sleaford, Spalding, Grantham, and Stamford.
• Commission services as needed, and understand the intricacies of care planning, direct payments, and signposting.
• Undertake mental capacity assessments and apply strength-based approaches to support individuals' autonomy and choices.
• Engage in Continuing Healthcare processes and participate in Decision Support Tools (DSTs) to secure appropriate care funding.
• Execute adult safeguarding practices to protect individuals at risk, ensuring their safety and wellbeing.
