What you will do:

- Provide a proportionate, timely social work service to adults with the appearance of care and support needs and their carers.
- Promote wellbeing through person-centred approaches, setting individual outcomes and empowering clients to achieve them.
- Prevent, reduce, and delay the need for support by utilising strengths-based practice models.
- Conduct robust safeguarding adults’ enquiries to protect adults at risk of abuse and neglect.
- Engage in risk management, collaborating with clients, their support network, and professionals to address any safety concerns.
- Undertake needs assessments, aligning support with statutory criteria and promoting independence.
- Play a key role in NHS Continuing Healthcare decision-making processes, working closely with multidisciplinary teams.
- Develop creative support plans with clients and carers, enabling them to utilise community and personal resources.
- Communicate effectively across all platforms, ensuring clarity and understanding among clients, carers, and professionals.
- Maintain comprehensive records, ensuring compliance with GDPR standards.
