What you will do:
- Conduct comprehensive assessments of needs and capacity for individuals in the community.
- Organise and lead multi-disciplinary team (MDT) meetings on TEAMS, ensuring effective collaboration and communication.
- Lead on safeguarding enquiry work, demonstrating your expertise and commitment to protecting adults at risk.
- Attend safeguarding meetings to provide valuable insights and recommendations.
- Travel to various locations, including Cubert, St Newlyn East, Perranporth, St Agnes, Porthtowan, Chacewater, Carnon Downs, Devoran, Perranaworthal, Perranwell Station, Ponsanooth, and St Day.
