What you will do:
- Manage a diverse caseload of adults with varying care and support needs.
- Conduct strengths-based assessments, care planning, reviews, and risk assessments, empowering individuals to thrive.
- Facilitate access to community resources, voluntary organisations, and commissioned services to maximise independence.
- Apply relevant adult social care legislation and statutory responsibilities with diligence.
- Collaborate with service users, families, carers, health professionals, and partner agencies to deliver holistic support.
- Maintain precise and timely case records to ensure transparency and continuity of care.
- Make informed and autonomous decisions in complex situations with competing priorities.
Requirements:
- Qualified Social Worker with Social Work England registration.
- Experience in Adult Social Care, demonstrating a thorough understanding of relevant legislation.
- Strong communication, negotiation, problem-solving, and decision-making skills.
- Capacity to work independently while contributing to a multidisciplinary team.
- Satisfactory employment references covering the last three years, with any gaps explained and verified.
