This is an exciting opportunity for an innovative and experienced Social Work Senior Practitioner to join our Next Steps Team within the Children in Care service, based in Pity Me, Durham, near the vibrant Arnison Centre. This full-time role of 37 hours per week offers the unique chance to make a significant impact on the lives of young people aged 16 and 17 as they transition towards adulthood.

What you will do:

- Manage a balanced caseload of young people aged 16–17, guiding them through the transition towards independence.
- Lead on pathway planning to help young people understand their journey, setting and achieving their future aspirations.
- Coordinate key transitions with residential and supported accommodation providers, ensuring smooth and well-timed moves that foster stability.
- Engage proactively with partner agencies, supporting seamless transitions and high-quality outcomes.
- Develop co-produced safety plans with young people, encouraging confidence, independence, and safe risk-taking.
- Build and maintain trusted relationships, providing security and emotional stability to the young people you support.
- Actively participate in reflective practice, group supervision, and development days to continually improve service delivery.
- Incorporate young people's feedback into practice, enhancing service development, and contributing to the innovative Super Hub initiative.
