What you will do:

• Conduct assessments and reviews as part of the Locality Team to ensure the best possible outcomes for social care service users.
• Develop and implement tailored support plans in collaboration with clients and other professionals.
• Work closely with a multidisciplinary team to ensure comprehensive, person-centred care.
• Manage a diverse caseload effectively, prioritising tasks according to urgency and importance.
• Offer guidance and mentorship to junior staff members and newly qualified Social Workers.
• Maintain accurate and timely records in compliance with legal and organisational standards.
