• Manage a caseload of rough sleepers, delivering tailored recovery and support interventions.

• Conduct comprehensive assessments, including substance misuse, housing needs and risk assessments.

• Develop and implement person-centred recovery plans, promoting engagement and positive outcomes.

• Deliver harm reduction and psychosocial interventions, supporting individuals with drug and alcohol dependency.

Person Specification:

• Relevant qualification in Health & Social Care, Substance Misuse, or equivalent is desirable.

• Evidence of continued professional development is essential.

• Knowledge of safeguarding procedures for adults and vulnerable individuals is required.

• Understanding of relevant frameworks, including homelessness legislation and substance misuse guidance, is crucial.

• Enhanced DBS clearance is necessary to ensure the safety and well-being of clients.
