Salary: £32 Hourly

What you will do:

- Be an integral part of the daily duty rota, handling crucial incoming calls and referrals with finesse.
- Conduct Care Act strengths-based assessments to understand and aid individuals’ needs effectively.
- Develop tailored Care Act care and support plans, ensuring regular reviews to guarantee quality care.
- Apply for essential funding for placements, supporting smooth transitions for vulnerable adults.
- Collaborate with health colleagues in creating Section 117 aftercare plans.
- Participate in multidisciplinary team (MDT) ward rounds and community meetings, bringing social care perspectives to the table.
- Plan and facilitate timely discharges from acute and psychiatric hospitals, ensuring a smooth transition for residents.
- Assume the role of Lead Enquiry Officer (LEO) in safeguarding enquiries, protecting vulnerable individuals.
- Carry out detailed Carers Assessments under the Care Act to support carers effectively.
- Conduct thorough Mental Capacity Act assessments and ensure best interest decisions are made.
- Engage in team meetings, contributing to a culture of development and support within the team.
