What you will do:

• Manage a complex Occupational Therapy caseload.
• Undertake strengths-based assessments to tailor interventions.
• Develop and implement person-centred reablement plans.
• Support individuals in regaining their confidence and independence.
• Provide specialist Occupational Therapy advice and interventions.
• Coordinate support with families, carers, and partner agencies.
• Manage risk to ensure positive outcomes.
• Promote evidence-informed Occupational Therapy practice.
• Contribute to service development and quality improvement initiatives.
• Undertake community visits to homes, hospitals, and other settings.

Additional Information:

• Hours: 37 hours per week.
• Working Hours: 08:45 - 17:15.
• Hybrid Working: Up to 50% of admin time can be completed remotely.
• IR35: Inside IR35.
• Fast-paced but supportive OT-led Reablement Team.

Requirements:

• Degree or Diploma in Occupational Therapy.
• HCPC registration.
• Minimum 2 years post-qualification Occupational Therapy experience.
• Reablement experience within Adult Social Care.
• Experience managing complex caseloads using strengths-based assessment experience.
• Strong risk assessment and clinical reasoning skills.
• Ability to work autonomously with excellent communication and partnership working skills.
• Full UK driving licence.

Desirable Requirements:

• Local Authority experience.
• Community Occupational Therapy experience.
• Experience supporting service development.
• Experience working within multidisciplinary teams.
