What you will do:

• Undertake Care and Support Assessments in line with the Social Services and Well-being (Wales) Act 2014, evaluating individuals' needs to create comprehensive care plans.
• Develop, implement, and review outcome-focused care and support plans tailored to the unique needs of adults requiring assistance.
• Manage a complex caseload, ensuring statutory responsibilities and timelines are met with precision and care.
• Complete safeguarding enquiries, coordinating with multi-agency teams to address concerns of abuse or neglect effectively.
• Conduct Mental Capacity Assessments, contributing thoughtfully to best-interest decision-making processes where required.

Roles and Responsibilities:

• Develop expertise in undertaking statutory assessments, reviews, and safeguarding enquiries to meet the diverse needs of service users.
• Support adults dealing with complex physical, mental health, learning disability, or age-related challenges, providing knowledgeable and empathetic care.

Person Specification:

Qualifications:

• Hold a degree in Social Work, CQSW, DipSW, or an equivalent recognised qualification, complemented by evidence of ongoing professional development in Adult Social Care.

Registration & Compliance:

• Maintain current registration with Social Care Wales, demonstrating commitment to high standards of professional practice.

Experience:

• Possess post-qualification experience within Adult Social Care services, demonstrating capability in statutory assessments, reviews, and safeguarding interventions.
