What You Will Do

• Conduct comprehensive assessments of adults and their carers, focusing on meeting their care and support needs in diverse settings.

• Undertake capacity assessments and facilitate Best Interest Meetings/decisions in alignment with the Mental Capacity Act 2005.

• Provide expert guidance and support to adults who may struggle with capacity, ensuring their accommodation and care needs are met with dignity and respect.

• Manage a varied caseload and deliver high-quality social work services to individuals with care and support needs, including those at risk of harm and neglect.

• Proactively identify vulnerable individuals at risk of significant harm and ensure appropriate safeguarding actions are taken, while respecting their right to self-determination.

• Collaborate effectively with a wide range of professionals and agencies within a multi-disciplinary framework.

• Maintain comprehensive and accurate records, providing relevant statistical information when needed.

• Advocate on behalf of adults to resolve conflicts and promote their best interests.

• Contribute to projects aimed at improving services for adults with care and support needs.

Qualifications and Experience

• A recognised Social Work Qualification (CQSW, CSS, DipSW, or Degree in Social Work).

• Registration with Social Work England.

• Experience in direct work with adults and within a multi-disciplinary team.

• A strong grasp of the Care Act 2014, Mental Capacity Act 2005, and relevant safeguarding policies.

• IT literacy and strong administrative skills are essential for record keeping and workload management.
