Role Purpose
As a Prevention & Wellbeing Social Worker, you will play a vital part in promoting independence, preventing escalation of need, and supporting adults to live fulfilled lives. Through early intervention, holistic assessment, and person-centred planning, you will ensure individuals receive the right support at the right time.
What You Will Do

Conduct holistic needs assessments and develop tailored support plans in line with the Care Act.

Apply robust safeguarding knowledge to protect vulnerable adults and manage associated risks.

Promote independence and wellbeing by connecting individuals with appropriate services and community resources.

Advocate for carers’ rights, ensuring their needs are recognised and addressed within care planning.

Maintain accurate, high-quality records using case management systems.

Work collaboratively with health, housing, voluntary, and multi-disciplinary partners to deliver coordinated, effective support.
Essential Skills & Experience

Social Work qualification with current professional registration.

Strong understanding of safeguarding legislation and best practice.

Experience using adult social care case management systems.

Excellent communication skills and a commitment to person-centred practice.

In-depth knowledge of the Care Act and its application in frontline practice.
