What you will do

Conduct comprehensive assessments of service users presenting with substance misuse and associated vulnerabilities.
Develop, implement, and review personalised recovery and resettlement plans to ensure continuity of care post-release.
Coordinate access to vital resources such as treatment services, healthcare, housing support, employment programmes, and more.
Maintain effective working relationships with important entities including prison healthcare teams, security departments, probation services, and partner agencies.

Person Specification

Qualifications:

NVQ Level 3 or equivalent in Health and Social Care, Substance Misuse, Criminal Justice, or a related field.
Evidence of continuing professional development in recovery, addiction, or offender rehabilitation.
Desirable training in safeguarding adults and trauma-informed practice.

Registration and Compliance:

Ability to obtain and maintain Prison Security Clearance.

Experience:

Track record supporting individuals affected by substance misuse, offending behaviour, or complex social challenges.
Experience working within criminal justice, prison, probation, community recovery, or related health and social care settings.
Proven track record of collaborating with both statutory and voluntary sector organisations.
