What you will do:
- Offer comprehensive support to individuals with life-limiting conditions, their families, and carers, focusing on holistic care.

- Create and review care plans, develop interventions, and assess emotional resilience.
- Lead on safeguarding, risk management, and addressing vulnerabilities such as mental health challenges and disabilities.
- Provide guidance on housing, benefits, financial matters, and access to community services.
- Write detailed reports for Long Term Care Benefits and coordinate respite planning.
- Facilitate communication within families and offer counselling referrals where necessary.
- Advocate for dependent adults following the loss of a caregiver.
- Manage a diverse caseload, engage in performance reviews, and contribute to raising awareness of social work within the community.
- Deliver training, education, and participate in valuable research initiatives.
- Ensure compliance with all legal, statutory, and professional standards, maintaining the highest level of service.
