What you will do:
- Assess the needs of clients referred to the Intermediate Care Team, developing bespoke intervention plans that promote independence and improve quality of life.
- Work collaboratively with a variety of stakeholders, including social services, healthcare professionals, and family members, to ensure the delivery of co-ordinated care and support.
- Utilise your expertise to recommend adaptations, equipment, and techniques to support daily living activities at home or in community settings.
- Document and maintain comprehensive case notes, recording patient progress and adapting interventions to meet evolving needs and circumstances.
- Participate in regular team meetings, share insights and strategies, and contribute to multidisciplinary discussions to ensure optimal care delivery.
